Most TLC tasks have one official form, and each form goes to one place. Some are uploaded in TLC UP, some are emailed to a named TLC inbox, and some must be handed in at a Long Island City appointment. Several common tasks have no paper form at all: new applications, renewals, fee payments and address changes all happen online in LARS. The table below maps each task to its form and where it goes, with links only to nyc.gov.
Everything here was checked on TLC's pages on October 12, 2026. Forms change, so open the link each time.

The driver forms, in plain words
Credential Replacement vs Request to Update TLC Credential. These two are easy to mix up. The Credential Replacement form is for a card that is lost, stolen, damaged, confiscated or "lost in mail." It is also used for name changes and, for drivers with an out-of-state DMV license, photo updates. The Request to Update TLC Credential form is the one TLC's FAQ names for name changes and new photos. If you hold a New York State DMV license, TLC's licensed drivers FAQ says to change your name or photo at the DMV first, because TLC now uses your DMV photo. Then you book a TLC appointment for a temporary license, and the new card is mailed. Both forms say a $25 fee applies. TLC's Credential Replacement Instruction Sheet lists the documents to bring as originals plus copies, such as a marriage certificate or naturalization certificate for a name change.
Advancement Request. This form moves your license expiration date earlier, once per license term, when you will not be available for the renewal drug test. The form says you must be in the third year of your license and must already have done your anniversary drug test. Details are in moving your TLC renewal date earlier.
Request to Cancel. One form covers base, vehicle and driver licenses. It says cancellation is permanent and the license cannot be reopened. A family member can sign only with documents showing why the licensee cannot. The form says a power of attorney is not accepted. A lawyer signs on letterhead.
DMV State Change affirmation. TLC's forms page says you must send it every time your DMV license moves to another state, including from New York to another state, and that failing to send it "may negatively impact your TLC License status."
Upload, email or in person?
Upload in TLC UP. Documents for an open application or renewal: the medical form, the defensive driving certificate, the FS-6T receipt, DMV records, court papers. Go to Menu → Requirements. The TLC UP portal guide shows the clicks.
Online in LARS. Applications, renewals, fee payments and contact changes. LARS has no upload box. See the LARS guide.
Email. Cancellations, base transfers, decal replacements, insurance updates and the DMV State Change affirmation. For the DMV State Change affirmation, TLC asks you to put the form name and your TLC license number in the subject line; doing the same for other emails makes your request easier to match.
In person, by appointment only. Credential replacement, name change and photo updates (for out-of-state licensees) happen at 31-00 47th Avenue, 3rd floor, Long Island City. Book "Driver License Replacement" in TLC's online scheduler. TLC's appointment pages warn that the phone number you book with must match the one TLC has on file, or the transaction will not be processed.
Forms that change often: check the date
Many TLC forms print a version date in the footer. Here are the dates on the copies we downloaded on October 12, 2026:
A newer date means the form changed recently. If the date on TLC's site is newer than your printout, use the new one. The out-of-state ID-5 step only started on October 1, 2025 (Get a TLC Driver License), and TLC says applications filed after that date are not processed until the FS-6T is uploaded. The full process is in out-of-state drivers: ID-5 and FS-6T.
Step by step: using a TLC form correctly
Mistakes that cost drivers money or time
- Booking an appointment with a different phone number. TLC says the transaction will not be processed.
- A stale medical form. The medical form says the exam can't be more than 90 days before you submit the application.
- Mailing an address change. There is no paper form. Use LARS, and do it within ten days of moving (rule 80-12(i)(3)). See your TLC mailing address.
- Thinking cancellation clears tickets. TLC's FAQs say cancelling a license does not remove open summonses or suspensions. The Request to Cancel form itself says cancellation is permanent. See cancel or surrender a TLC license.
- Returning FHV plates without a plan. Surrendering plates ends an FHV license unless you are entering storage.
Points on your DMV record are not changed by any TLC form. See DMV points for TLC drivers.
Forms from other agencies
Some paperwork drivers need is not a TLC form:
- Crash report to the DMV. See do I need to file an MV-104 after a crash.
- Lost or stolen plates. TLC's transfer page says to get a police report (MV-78B) from the NYS police if both plates are lost or stolen, then surrender at a DMV office for an FS-6T receipt.
- Tax forms from the apps. See will I get a 1099-K from Uber in 2026 and the Form 1099-K glossary entry.
- Drug test. No form to fill. TLC lists LabCorp at (800) 923-2624. See TLC drug test.
Not sure which license you even need? Try which TLC license do I need. The medical form is explained line by line in the TLC medical certification form. More on the licensing hub.

Frequently asked questions
Where are TLC forms?
On nyc.gov. TLC has a 'Frequently Used Forms' page for drivers, and the other forms are linked from the page for each task (renewal, transfers, insurance, FAQ pages). Every official form is a PDF under nyc.gov/assets/tlc. Download forms only from nyc.gov, not from other websites.
Is there a TLC change of address form?
No paper form. TLC's Frequently Used Forms page says to log in to LARS for a change of address. Each TLC license you hold must be updated separately.
Which form do I use if my TLC license never arrived in the mail?
The Credential Replacement form (req_driver_replace_credentials.pdf), which has a 'Lost in mail' box. You submit it at a Long Island City appointment after waiting 10 business days from the mailing date shown in TLC UP. The form says this replacement is free if 10 to 60 days have passed since TLC mailed the card; otherwise the fee is $25.
How do I send a TLC form?
It depends on the form. Some are uploaded in TLC UP (medical form, FS-6T receipt), some are emailed (cancellation, base transfer, decal replacement, DMV state change, insurance), and some must be brought to a Long Island City appointment (credential replacement, name or photo update). The table on this page shows each route.
Do TLC forms change?
Yes. Many TLC PDFs print a version date in the footer, for example the Request to Cancel form dated 11/28/2025 and the DMV State Change affirmation dated 10/20/2025. Download a fresh copy each time instead of reusing an old printout.
